Canadian Entry Authorization Form

For groups traveling to Canada, it is required that travelers under the age of 18 present a Canadian Entry Authorization Form at customs, indicating that the participant has permission to travel with the group. A parent or guardian must complete this form. Travelers must bring this Canadian Entry Authorization Form with them on tour. Please do not send it to EF.
