Canadian Entry Authorization Form

For groups traveling to Canada, it is required that travelers under the age of 18 present a Canadian Entry Authorization Form at customs, indicating that the participant has permission to travel with the group. A parent or guardian must complete this form and have it notarized no earlier than 30 days prior to departure. Travelers must bring this Canadian Entry Authorization Form with them on tour.
